Essay on Patient Protection and Affordable Care Act

PATIENT PROTECTION AND AFFORDABLE CARE ACT Tammy R. Carr Grand Canyon University: HCA-675 April 2, 2014 Patient Protection and Affordable Care Act The Patient Protection and Affordable Care Act was signed into law by President Barack Obama in March of 2010. This law provides equal access to medical care, lowered health care costs and eliminates denial of coverage of pre-existing conditions to the millions of the uninsured and insured Americans that were without and denied health care coverage. Patients who were denied coverage due to pre-existing conditions can now look forward to relief and great improvement because their illness is covered in the new policy, and care is now provided for them at next to minimal cost (Stehly,†¦show more content†¦The Affordable Care Act is providing medical care at an affordable rate and can decrease extra stress and strain on families that struggle and have previously tried to meet their basic needs, let alone high medical debt. According to Medicaid.gov, Children’s Health Insurance Program (CHIP) provides quality medical coverage to families that can’t qualify for Medi caid or private insurance (Services, 2014). One of the major advantages of having insurance for children and teens is having the benefit of having access to vaccinations. Disease prevention is the key to a happy and healthy start. If you have healthy children, it can be a plus for their learning in school by reducing unnecessary days out of school because of illness. By providing access to medical care, it also reduces unnecessary emergency department visits. One of the cons of the PPACA could be the tax penalty for not carrying insurance coverage. The penalty starts at $95 or 1% of income, whichever is greatest. If the family or individual still chooses not to carry insurance, the penalty will rise each year. Analysis of Themes in the Tell-Tale Heart by Edgar Allan Poe

There are themes in every piece of fictional literature ever written. A theme is the central idea of a story that is fictional. A theme can be everything from good verse evil to as simple as light and darkness. In any story there may be more than one theme in it. Some stories have numerous central ideas that can be seen in the one. Most people only focus on one while there may be five that are important to understand to understand the story. The Tell-Tale Heart like some has numerous themes that are all important to understanding the story. One of the theme’s more prevalent themes that present it’s self in the Tell-Tale Heart the theme of is insane verses sane. This theme is one of the central themes in the story. You can see this in the first sentence of the story in which the person says â€Å"True!—nervous—very, very dreadfully nervous I had been and am but why will you say that I am mad† (Poe, 331). The more the man tries to convince the people he is retelling the story that he is sane the more it shows how very much insane he actually is. When he tells the story of the old man that he murdered he tells it calmly and remorseless. He states in his retelling that he did not hate the old man or that he wanted the old man’s wealth when he murdered him. “Freeze!” †Creative Writing Free Essays

The loaded fork stopped short of the man’s mouth. His eyes scanned the room to find out where the shout came from, his eyes followed to the door of the room and stopped. There in the doorway he could make out four officers, guns ready should he move unexpectedly. He thought better of it and put the fork, still loaded on to his plate. Slowly he raised his arms in the air. Officers streamed into the cramped room, surrounding the man there were as many as could fit with space enough to raise a gun. Some officers were what seemed to be too close to the table, but they could not retreat for the officers behind them. One man passed out from the sight, he was dragged out before he caused a commotion. The seated man smiled at the officer’s weakness and followed his limp body out of the room with his eyes. There continued a small bustle as the man was arrested, read his rights, cuffed, masked and shackled. He was walked out of the room, up the flight of stairs that led from the basement flat and out in to the street, where he could see how important he was. Squad cars, and two SWAT vans littered the already dark street. The man could just make out the head of the police department sat comfortably in his black, government car. The man was shoved into the back of an armoured van which already contained three well built men. They were fully equipped with batons and cattle prods. The man decided it best to sit quietly and enjoyed the ride in the windowless van. He shut his eyes and dreamed as he swayed with the jolts the dirt road gave the van. What seemed like hours had passed when the van finally stopped. A minute later, the doors opened and the man was pulled out, still cuffed, masked and shackled. He was escorted into a large important looking building filled with blue uniformed men. The man was led through a maze of corridors and pass checks. They came to a narrow, brightly lit, warm corridor with sound proofed walls and bullet-proof Perspex panels and doors that opened in on to neat holding cells. He was taken down to the last cell on the left, even though all the other nine cells were empty. His cuffs and mask were taken off but they left the shackles, he was politely asked to step inside the cell. He thanked the two officers that had been left with him and walked into the cell. The door was locked behind him. The officers out of duty asked if he wanted anything. â€Å"I’m a bit hungry.† The blood drained from the younger officer’s face. â€Å"I was wondering if I could finish my meal?† From the look on their faces he could tell the answer was no. â€Å"Maybe I could have a snack, a packet of crisps or something?† Their faces seemed to relax. â€Å"What flavour?† Quested the young officer. â€Å"Bacon if possible.† The man saw the young officer recoil, the older officer seemed better trained and showed no emotion. â€Å"We’ll see what we can do.† The older officer stated. â€Å"Thank you,† the man responded, â€Å"By the way, do you know how long I’m going to be here? I mean will I just have to wait for a little bit or will I have more time that I could waste sleeping or something?† â€Å"We don’t know. Do what you see fit.† The older officer motioned the younger officer to follow and they walked out of the man’s view. The man settled down and took in his surroundings. Three white walls and one Perspex panel and door held him captive. There was a smooth cot bed made up too perfection with cream blankets and sheets. A steel toilet stood quietly in the corner with a small matching basin next to it. Nothing else furnished the room. The man sat down on the floor and leaned against the cot. With the cool metal side of it in his back he knew he would not be comfortable in this position for long, but he was too lazy to move yet. The man shut his eyes and began to dream. His dreams were plagued by food for he was hungry. He had been interrupted during his dinner and so his belly was not full or content. He dreamed of a starter of soft Cornish crackers, smothered with liver pà ¯Ã‚ ¿Ã‚ ½tà ¯Ã‚ ¿Ã‚ ½. For the main course a beautifully cooked meat pie, soaked in gravy with a very small salad on the side. For dessert a tray of mince pies. He always wondered why they never actually had any real mince in them but even so he delighted in eating them, just for their name. Just as he finished his first mince pie and was about to reach out for another, a soft knocking on the door interrupted him. He opened his eyes to the older officer standing outside the cell staring at him. The man stood and stepped forward towards the glass. The officer stepped back and from his pocket produced a shiny packet. â€Å"I only have roast chicken flavour left.† The man looked at the officer puzzled. â€Å"It must have been confiscated from a convict, nobody will miss it I assure you.† The officer spoke too quickly. He placed the packet in the food tray on the floor and slid it shut so that the drawer’s content was now on the inside of the cell. The man picked it up. â€Å"Thank you very much. I wasn’t expecting anything at all. Thank you again.† The man sat on the cot and proceeded to open the packet of crisps. He placed the bag close to his face and smelled its contents. He looked back at the glass and was surprised to see the officer still there. He smiled and pulled out a wafer of potato. As he crunched into it, the officer turned and walked away, the man thought he saw a smile on the officers face but could not be sure. It almost looked as if the officer was longing for something, but the man dismissed that idea, why would an officer long for a packet of crisps? The officer probably was feeling sorry for the man. The man ate half the packet and decided to save the rest for later. He put the packet under the cot pillow. He kicked off his shoes, imitation leather, slid on to his side and drifted off to sleep on the cot. A long while later, he was woken up by a buzzer he could not see. He stood in time to see four officers at his cell. He was led through another maze of corridors and pass checks to a small questioning room. He was sat down at a medium sized wooden table with two chairs, one that he occupied. A large mirror took up the wall opposite where the man sat. He suspected it to be a two-way mirror for obvious reasons. He was left there with the shackles clinking softly at his ankles. Two officers entered around a half hour later. One sat down, the other stood in the shadow near the door. The seated officer placed a Dictaphone on the table, switched it on and stated the date, time and the names of all present in the room. The man was shocked that it had been a full 12 hours since he had been caught. Then it began; the barrage of questions. First they started slowly; his name, age, date of birth, height, weight and so on. They gave him time to answer each question fully and only asked another when he looked as if he had nothing more to say. Then they began to speed up. â€Å"When was the last time he had food?† He told of the crisps, interrupting they asked what flavour. â€Å"Roast Chicken.† â€Å"Who gave them to you?† â€Å"Another officer, I don’t know his name.† â€Å"Do we look stupid?† The man did not answer. â€Å"No officer is allowed to give you food.† â€Å"But-† â€Å"No officer would give you chicken flavour crisps.† The man put his head down as in defeat. â€Å"And before that? What was the next before last thing you ate?† â€Å"Lunch.† â€Å"Consisting of?† â€Å"Meat.† The man had raised his head to look the officer in the face before he had replied. A look clouded both officers’ faces. The seated officer rose and motioned the other to sit. This officer was dressed differently, for one she wore a dark blue skirt rather than trousers. Instead of the usual white shirt the men wore, she wore a light blue blouse and a small tailored dark blue waistcoat. â€Å"This, † motioned the now standing officer, â€Å"is officer Ali, first name Aliya. She is very familiar with your type of problem.† â€Å"Hello.† A quiet, slightly shaky voice indicated she was not as good as her introduction. â€Å"Hello Miss.† The man said politely. â€Å"Address the officer properly.† â€Å"Nice to meet you officer Ali.† â€Å"That’s okay officer. Let the man relax.† The standing officer grunted indignantly and walked towards the door to lean against the wall near the light switch. â€Å"Okay. Let’s start.† The young lady produced a bunch of papers from a small briefcase by her feet. â€Å"These are just a few character tests I need you do to so that I can help you. Would you mind taking one of these?† She looked at the officer for approval. He nodded. â€Å"Not at all officer. I know I’m sick and need help. But I haven’t the will power.† The man looked genuine in his feelings. â€Å"Okay. Right. Okay. How often do you eat meat?† She laboured over the last word, as if it was uncommon. â€Å"Not very often only when I can find some. Then I tend to gorge myself on it.† â€Å"And where do you get your†¦meat?† She fuddled over the word again â€Å"An out of town butcher† The man said flatly. â€Å"Really?† She raised her eyebrows in surprise. She suspected he was lying. â€Å"No. I catch it myself. I stalk them and then kill them with a knife, that way not bullets or poison spoils it.† He had lied and had thought better of it to tell he truth. He put his head down again in shame. â€Å"Spoils it?† Her eyebrows now knit together in curios â€Å"The taste is better if there isn’t all that stuff.† He was a little cautious of his answers but he was trying to be as honest as he could, he wanted help and they could only help him if they knew everything. â€Å"Ok. Do you think you could ever stop†¦doing what you are doing?† She struggled again to ask these questions. This was new to her, she had only been doing this for about a month and this was the first man who was this honest. The rest of the offenders usually laid the whole way through. She knew why; they had heard of what would happen to them if they were convicted. â€Å"I hope so. I can go without it for long, weeks at a time but then a craving for it plagues my mind and body.† He looked for some kind of reaction in here eyes. Another inquisitive look knotted hey eyebrows together. â€Å"You have withdrawal symptoms?† â€Å"Yes. I start, dreaming. I’m a good cook. I dream up dishes that I can make. I can make anything out of any bit of meat.† He sounded slightly proud of himself. But then he remembered why he was there and retreated back to feeling sorry for himself. â€Å"Can I ask why you call it meat? Not flesh, or food?† â€Å"If you think of it as what it really is, then you begin to feel guilty. I don’t like feeling bad, so I don’t think of them as anything important, only as bits of meat that taste nice.† It was an answer given matter-of-factly. â€Å"If we put you on a diet of only vegetables would you be able to survive?† She looked for a way out for him, she felt sorry for him. â€Å"Yes of course, I wouldn’t like it though.† This was not going well for the man. She asked the final question that she knew would decide his fate. She wished he would answer correctly but she could give no kind of indication to him as to what his answer should be. â€Å"If you had the chance to have meat again after this interview would you take it?† A slight pause as the man put his head down and answered n a small voice. â€Å"Yes.† Suddenly the door burst in and armed guards rushed into the small room. In desperation the man realised that the rumours of what happened to people like him were true and that he had just sealed his fate. â€Å"I mean no, I wouldn’t. I wouldn’t! I promise, no, no I wouldn’t.† He screamed apologetically, begging for forgiveness and promising that he would be good. He sounded like a little boy begging his parents for one more chance after doing something wrong and knowing he would be punished. The office seated stood up and walked out of the room. She looked back at the man with sympathy but she knew she could do nothing. He had answered wrong. He was dragged out of the room by guards who had been waiting outside the door. â€Å"No! I promise, never again!† The man was taken to a cell not too far from the questioning room. He cried and screamed, kicked and bit. He tried everything to free himself. It didn’t work. He was thrown into a small cell, 4 foot by 3. Blood was spattered on the walls and the man shook as he realised in his mind what was actually going to happen. Three small flaps near the floor opposite the now bolted steel door snapped open. Out of them came a stream of fat brown rats. They circled him, ring upon ring formed round him, the man stood still, his eyes racing round to each small set of beady eyes. Their bright yellow teeth were visible and chattered every so often, all daring each other to jump first. A silent sign made them all pounce on him. He shrieked and tried to pull them off. They bit through his shirt, on his bare arms and at his legs. Many drew blood on their first bite. These were well-trained assassins. They were hungry and wanted meat. The man’s shoes had been kicked during the scrap and now the rats bit at his socks. The man fell on the floor and they swarmed him. Wave upon wave of pain coursed through the man. They were biting all over his body, regardless of whether it was clothed or not. If it were clothed, they would slash an opening and clamber through to the soft skin underneath. The rats were in his shirt, in his trousers biting at everything. The man continued to fight them off, but there were just too many. He kept screaming until one rat dared to jump into in and bite his tongue. The man bit down as hard as he could. Half the rat fell off of him, tail still twitching. He spat the other half out and spat out the rodent’s blood, it mingled with his own from the bite on the tip of his tongue. He stopped screaming but when a rat latched on to his nose he yelped in pain. It’s tail swung into his mouth and he bit it off accidentally swallowing it. Another rat bit down into the flesh between his thumb and fore finger, he tried to shake it off but the momentum plus the weight of the rat caused the flesh to rip and the rat to fly off with part of the man in it’s mouth. It dropped the meat and attacked him again on his other hand hoping for a larger piece. Rats on the man’s belly ripped and clawed their way past the skin, getting to the soft meat beneath it. Five especially fat rats tunnelled through the man into his abdomen and chest. The man could not breathe, as his lungs were half eaten. He coughed up blood and in it was a bit of a rat’s tail. The man retched but nothing came up as his stomach was on the floor in front of him. Eventually the man ceased to move. His efforts for survival had been futile. The rats continued their meal until all that was left was the bones of a man. When they were full they sat in the corners of the room letting the still-hungry rats gorge till they themselves could eat no more. Hours passed and slowly the rats scampered back to their homes on the other side of the flaps, where more food was. When every rat had gone, two officers shut the flaps, opened the door and collected the remains. They remarked about how efficient the rats were getting, this one had died within two hours of being locked in. The other remarked the time was too short and that every non-vegetarian should endure the most torture they could receive before dying. They both agreed that all meat eaters should be put to death and that this was the most fitting way for it too be done but they still continued to argue over how long it should last. Their argument lasted all the way to the dump where they threw the filled bin liners into a newly dug pit, big enough for thirty bags or so. By then they had grown tired of the discussion and were now debating whether to go to the new veggie-restaurant, or go to the usual veggie noodle bar for lunch. School uniforms may help families but some students may disagree that it’s a way to take away ones individuality. School uniforms will restrict the outward expression of a student’s individuality. Some students thrive on individuality with uniforms they can’t. Although some students don’t think uniforms are right the schools main priority is to educate and in order to do this the school must have a safe learning environment. Second some students think that uniforms make them blend in. to illustrate â€Å"schools primary function is to educate, but secondary is a platform for socialization where clothing can play a role and with uniforms this makes it difficult for students to stand out† (Thomson 1). Students’ self-esteem may go down because they can’t wear their own clothes. However for other students it creates a sense of belonging. Even though students may think that uniforms take away their freedom of expression it is the schools responsibility to educate with a safe and welcoming environment. Research shows that students can achieve success by wearing uniforms therefore it is a great idea to enforce a policy on school uniforms. If schools everywhere were to introduce a school uniform policy there would be a dramatic change in students would feel motivated to go to school because they feel like they’re welcome. Dents Disease: Symptoms, Genetics, and Treatments Dents Disease Alessander Leyendecker Junior Clinical Aspects Dents disease is a recently characterized syndrome caused by a genetic mutation that leads to a renal tubular disorder and is characterized by manifestations of proximal tubule dysfunction. The proximal tubule is responsable for the reabsorption of sodium, potassium, calcium, glucose and low-molecular-weight proteins (like retinol binding protein and ÃŽ ±1 microglobulin and ÃŽ ²2 microglobulin) and for the secretion of ammonia, toxins, drugs and H+. Therefore, a proximal tubule dysfunction like Dents disease is characterized by low-molecular-weight proteinuria (excretion of low-molecular-weight proteins in the urine), hypercalcinuria (high concentration of calcium in the urine), nephocalcinosis (deposition of calcium salts in the renal parenchyma), nephrolithiasis (formation of crystalline aggregates that can develop anywhere along the urinary tract) and progressive renal failure. Low-molecular-weight proteinuria represents the most common symptom of Dents disease (Devuyst Thakker, 2010). Mapping: A higher disease severity in males and a lack of male to male transmission were identified by an analysis of 5 unrelated British families. The fact that females may show a milder phenotype suggests an X-linked recessive inheritance. The locus position on chromossome Xp11 was identified by linkage analysis of 2 3-generation families with Dent disease by Pook et al. (1993). Twenty X-linked polymorphic markers were used and the Linkage was established between Dents disease and the Xp1l 1 loci ARAFI (Z max = 5.42, 0 = 0.000), DXS426 (Z max = 3.61, 0 = 0.000), DXS255 (Z max = 5.48, 6 = 0.000) and DXS988 (Z max = 4.25, 0 = 0.045) to determine a locus order. In addition, DXS255 has presented a microdeletion in the affected members of one family, allowing the localization of Dents disease to Xp11.22 (Pook et al., 1993). Furthermore, another proximal renal tubular disorder associated with nephrolithiasis in a North American family and referred to as X-linked recessive nephrolithiasis has also been mapped to Xpl1.22. Suggesting that maybe there is one gene causing both X-linked recessive nephrolithiasis and Dents disease. Molecular Genetics: A study conducted by Fisher et al (1994) found a microdeletion in one Dent’s disease kindred, allowing the identification of the gene CLCN5 as a candidate. Furthermore, an investigation of 11 kindreds with renal tube disorders indentified 3 nonsense, 4 missense, 2 donor splice site mutations, 1 intragenic deletion and 1 microdeletion in CLCN5 gene (Llyod et al., 1996). Furthermore, Hoopes et al. (2005) indentified that some patients with Dents disease suffer from extra-renal symptoms such as cataract, mild intellectual impairment and hypotonia. Shared mutations in OCRL1 with the oculo-cerebrorenal syndrome of Lowe were identified in these patients. The presence of such extra-renal symptoms with mutations relating to Lowe syndrome is known as Dent disease 2. A research conducted by Bà ¶kenkamp et al. (2009) showed that low-molecular-weight proteinuria was a constant feature in all both Dent disease 1, Dent disease 2 and oculo-cerebrorenal syndrome of Lowe. However, there was a wide overlap in most of the other symptoms of the proximal tubulopathy. Nephrocalcinosis was observed more frequently in patients with Dents disease 1 than those with Dents disease 2 and Lowe syndrome; aminoaciduria, renal tubular acidosis, and renal failure was observed more frequently in patients with Lowe syndrome than patients with Dent-disease 1 and Dent disease disease 2. Genetics: All these findings have demonstrated that both Dent disease 1 and Dent disease 2 are a X-linked recessive inherited condition, caused by mutations in the OCRL1 (Dent disease 2) or CLCN5 (Dent disease 1) genes. These genes are located on chromosome Xp11.22 (Dent disease 1) and Xq25 (Dent disease 2). Mutation in CLCN5 are responsible for about 60% of the cases of Dent disease and mutation in OCRL are responsible for about 15% of the cases of Dent disease (Hoopes et al., 2004). So, nearly 25% of the cases of Dent disease do not have a known disease-causing mutation in OCRL or CLCN5, implying another genetic heterogeneity. Due to the fact that Dents disease is a X-linked recessive disorder, the disease is generally found in males only. Females carriers may show a milder phenotype. Furthermore, the disease may be present in early childhood (Devuyst Thakker, 2010). To date around 250 families with Dent-1 disease and about 50 patients with Dent-2 disease have been reported (Ludwig, Levtchenko Bà ¶kenkamp, 2014). The diagnosis of Dents disease is always difficult due to the wide variability of clinical presentation and, occasionally, lack of family history. Therefore, the disorder is likely underdiagnosed and the prevalence of this disease in the population is still unknown. More than 200 Dents disease 1 patients withCLCN5defects have been described with several different disease-causing mutations all over the gene. The types of mutations includes missense (44%) and nonsense (26%) mutations, small deletions/insertions (15%) and splice defects (11%), with a few hotspots, usually affecting arginine codons. Large insertions/deletions can be detected in around 4%of the patients. However, OCRLmutations in Dents disease 2 patients are not uniformly distributed. Missense mutations are usually found in exons 8–15 while nonsense or frameshift mutations very often affect exons (Ludwig, Levtchenko Bà ¶kenkamp, 2014). CLCN5 encodes the electrogenic Clà ¢Ã‚ Ã‚ »/H+ exchanger ClC-5. OCRL1 encodes a phosphatidylinositol bisphosphate (PIPà ¢Ã¢â‚¬Å¡Ã¢â‚¬Å¡) 5-phosphatase and mutations are also linked to the Lowe Syndrome. Mutations in any of these genes can culminate in the lack or dysfunction of these important proteins and result in the phenothype of Dents disease. Pathophysiology: The ability to reabsorb low-molecular-weight proteins and albumin that are ultrafiltered by the glomerulus are characteristics of the epithelial cells covering the proximal tubule portions of the nephron. This ability involves a process called receptor-mediated endocytosis. In the receptor-mediated endocytosis the particle to be endocytosed binds to specific receptor proteins concentrated at certain locations in the plasma membrane. Several types of receptors participate in the receptor-mediated endocytosis. These sites form a small depression in the plasma membrane that is covered by clathrin (a type of fibrous protein). After the binding of a ligand to plasma membrane-spanning receptors, a signal is sent through the membrane, leading to membrane coating, accumulation of the ligands into coated pits, and formation of a membrane invagination. The receptor and its ligand are then opsonized in clathrin-coated vesicles. The clathrin confers stability to the vesicle that are being transp orted into the cell. In the cytoplasm the vesicle loses its clathrin coverage and individual vesicles fuse to early endosomes. The acidification of endosomes by proton pumps ATP-dependent leads to the dissociation of the protein-receptor complex. This process allows the endosomes to fuse with the lysosomes through late endosomes. This acidification is reached by ATP-mediated transport of cytosolic H+ through the V-ATPase and demand an equal Cl conductance to preserve electroneutrality. It has been theorized that the H+ gradient can be neutralized by the C1C-5 action. In theory, ClC-5 provides an electrical shunt to neutralize the H+ gradient. Therefore, the vesicular acidification should be impaired by the loss of the endosomal Cl conductance mediated by ClC-5, resulting in the dysfunction of proximal tubule cells. To confirm this hypothesis, Piwon et al. (2000) created two distinct strains of ClC-5 knock-out (KO) mice which both have symptoms of the main characteristics of Dentâ₠¬â„¢s disease including manifestations of proximal tubule dysfunction like low-molecular-weight proteinuria. A reduced acidification of early endosomes in ClC-5-deficient mice have been demonstrated by in vitro experiments. However, ClC-5 is a 2Cl/H+ exchanger and not just a Cl channel, so the importance of this exchange activity for Dent’s disease was still unknown and needed to be assessed. In order to investigate this relevant question, Novarino et al. (2010) created a knock-in (KI) mouse presenting a point mutation in a important glutamate residue which transforms the exchanger into an uncoupled Cl channel that should help the endosomal acidification. The conventional ClC-5 KO mouse was subsequently compared with the KI mice. Acidification of the renal endosomes from wild-type and KI mice was regular, but greatly impaired in KO mice. Nevertheless, the same symptoms were indentified in both KI and KO mice, despite normal endosomal acidification and patients with Dentâ€⠄¢s disease, including low-molecular-weight proteinuria, hypercalciuria and hyperphosphaturia. Moreover, both the KI and KO mouse presented impaired proximal tubule endocytosis, indicating that proximal tubule dysfunction in Dent’s disease may happen in spite of the regular acidification of the endosomes. These findings indicate a function for a diminished endosomal Cl accumulation in Dent’s disease. Diagnosis: The clinical diagnosis of Dent’s disease is grounded on the presence of low-molecular-weight proteinuria, hypercalciuria, and at least one of the following characteristics: kidney stones, hematuria, nephrocalcinosis, renal insufficiency or hypophosphataemia (Hoopes et al., 2004). The clinical diagnosis can also be supported by nephrolithiasis and/or history of X-linked inheritance of renal Fanconi syndrome. The diagnosis can be confirmed by the recognition of mutation in both CLCN5 or OCRL1 by sequence analysis. Sequence analysis of affected males allows the identification of whole-gene deletions, multiexonic and exonic due to the lack of amplification is this case. Confirmation may also require deletion/duplication studies. It is imperative to previously identify the disease-causing mutation in the family when conducting a carrier test for at-risk relatives. The diagnosis of Dent disease 2, related with OCRL1 mutations, is more common than a diagnosis of Lowe syndrome when th ere are an absence of clinical symptoms and lack of severe intellectual deficit. Treatment: The reduction of hypercalciuria, prevention of nephrocalcinosis and kidney stones, and retardate the evolution of chronic kidney disease (nephrolithiasis) are the objectives of the current treatment. Thiazide diuretics in doses higher than 0.4 mg/kg/day have reducted urinary calcium excretion by more than 40% in males with Dent disease (Raja et al., 2002). Similarly, we need to be cautious due to the fact that an increased hypercalciuria can result from the treatment of rickets with vitamin D. Furthermore, recent studies has been shown that a high citrate diet can slow the progression of chronic kidney disease in CLCN5 knockout mice (Cebotaru et al., 2005) and can be efficiently used to treat Dent disease. Genetic Counseling: The two types of Dent’s disease are X-linked. Therefore, men are affected more harshly. The father of an affected man will not have the disease or will he be a carrier of the mutation. The probability of transmitting the disease allele in each pregnancy is 50% if the mother of the proband is a carrier. Males who inherit the mutation will be affected and females who inherit the mutation will be carriers and will generally not be strongly affected. Affected men pass the disease-causing mutation to all of their daughters (who become carriers) and none of their sons. THE REAL NATIONAL TREASURE The United States, along with the allied forces of the world, is currently fighting to find Osama bin Laden as well as his organization, al Qaeda, in Afghanistan. The new declaration of the â€Å"War on Terrorism† has withdrawn all boundaries that the terrorist organizations have been hiding behind. Although this war is a new idea to the average American Citizen, it is an old foe of the governments in most countries, which have already established antiterrorism and counter-terrorism task forces, but are now working together under an allied force. With the tightened security at the airports and military facilities, the counter-terrorism efforts are fighting back to counter the terrorists actions, but the key to fighting terrorism is to prevent the attacks from happening at all.   Ã‚  Ã‚  Ã‚  Ã‚  More than a majority of the American public supports this military action against countries that terrorize the United States. In fact according to a gallop poll, eighty-eight percent of Americans polled agree that the United States should take military action in retaliation for attacks on the World Trade Center and the Pentagon. The problem that is being faced by the world is how to avoid future attacks after the smoke clears from the current engagement.   Ã‚  Ã‚  Ã‚  Ã‚  There are two types of forces that are currently employed against terrorism and both have the same mission of fighting terrorism, with different applications. The antiterrorism force is used as a preventive action against future attacks while the counter-terrorism task force has many more post-action responsibilities. According to the Office of Counter-Terrorism, there are four policies being implemented as guidelines: â€Å"Make no concessions to terrorists and strike no deals, bring terrorists to justice for their crimes, isolate and apply pressure on states that sponsor terrorism to force them to change their behavior, and bolster the counter-terrorism capabilities of those countries that work with the U.S. and require assistance†. These steps are necessary to fight terrorism after the actions of horror have occurred, and the current Afghanistan action in search of the al-Qaeda members (including bin Laden) is an example of a counter-terrorism action.   Ã‚  Ã‚  Ã‚  Ã‚  In order to prevent future attacks, the United States must put a stronger emphasis on strengthening the antiterrorism forces. Putting more numbers on the membership board of this force is what is going to make headway in the â€Å"War on Terrorism†. It is a lot more difficult to fight terrorism before the attacks have taken place, but history has already demonstrated that the oldest form of intelligence collection, human intelligence, can be used to fight an entire country.

After reading through my cover letter and resume, the student advisor looked up and said, â€Å"This looks really good. In fact, we’re going to use this as an example for other students to follow. Nice work!† That wasn’t the response I expected, but I was glad to take it. This is a 100% true story, and I’m not sharing it to boast, either. Instead, Im sharing this experience to show that  getting the job you want in this industry is an attainable goal. And it all starts with your resume. Using people as references without permission is a major faux pas. Location. Location. Location. This step matters most if you’re applying for a job in another state or city, and will need to relocate. There are varying schools of thought on this, but this post will argue it’s best to be honest. Consider the following: If the job is within a commutable distance, use your own address. If you’d need to travel a longer distance for an interview, state that in your cover letter. If you can, consider using a friend or relative’s address in the city. Formatting Your Resume So, what should this thing look like? It’s an important question, and there are a handful of minor tweaks you can implement to improve formatting. Step 1. Pay Attention to Font Choices There are two types of fonts to pay attention to here: serif  and sans-serif. That’s why I used a sans-serif font on mine. Too many fonts is considered a negative for readability. Again, mine only uses one font (just in different formatting and sizing here and there). Using serif fonts isn’t going to ruin your resume if you’d really like to use them. You’ll probably still get hired. But, for ease of reading and skimming, limit yourself to two fonts, and lean toward sans-serif. Recommended Reading: How to Work With Designers With Authentic Advice From 's Designer Step 2: Consider Narrower Margins for Wider Sentence Length Resumes need to include a lot of information in very little space (ideally, within one page). One way to squeeze in more detail about yourself while retaining a clean look is to use narrow margins. This helps you fit more text per line. If you’re using Microsoft Word, you can easily set wider margins by clicking on the Layout tab: Next, click Margins. Then, select Narrow: This is how the template included in this post sets its margins. Step 4: Use Bold Text Selectively Bolded text can help important elements of your resume stand out. Consider bolding items such as: Your college and major/minor. Previous companies and job titles. That’s about it. Too much bolding can easily look obnoxious. Writing Your Resume You’re finally ready to start doing some actual writing. Here’s how to make sure you nail it. Only list your high school if you didn’t attend college (no judgment hereI know people who have done well in digital marketing without attending or going into post-secondary education). Include the following: The type of degree you received: This means a Bachelor of Arts (BA) or Bachelor of Science (BS), Master of Arts (MA) or Master of Science (MS, or an Associates (AA). Your major and minor: This may be formatted differently depending on the details of your degree programs. For example, my degree was in Mass Communications, with an Emphasis in Print Journalism and Public Relations. Next, list your college:  Also include the month and year you graduated (or plan to graduate). A word about grade-point averages: in my experience, most marketing hiring managers don’t prioritize them. While high academic achievement reflects well on your work ethic, it’s your skills that are going to get you hired. So, should you bother listing your GPA? Once you have the basic information out of the way, add your responsibilities and accomplishments. While a hiring manager will want to know what you had on your plate at a past job, they’ll be more impressed with what you actually achieved. So, make your experience stand out by doing this: Make it clear how your work positively impacted the business. Romeos friend Bonvolio had made his plan work. He planned to get Romeo to experience other beauties, or compare other women to his beloved Rosaline. This is also where the feud starts with Romeo and Tybalt because Tybalt is a Capulet and Romeo is a Montague. Tybalt is even angrier when he hears Romeos voice at his uncle Lord Capulets party because no Montagues are allowed. This by his voice, should be a Montague, Fetch me my rapier. The motivations that happen in the play show that the characters are doomed from the beginning because the forces greater than themsel ves. The forces include a long-running feud between the Montagues and the Capulets, something from which these lovers cannot escape no matter how hard they try.This scene has a great affect on all the other scenes in the play therefore I will be looking at how it is important. I will also be looking at how Shakespeare creates tension and interest for the audience.Act 1 Scene 5 comes at the end of the first act; this is because the audience are fully aware of the characters and their backgrounds. In this scene everyone and everything has been introduced to the audience and this tells us what will happen in the rest of the play. This scene also has dramatic irony because the audience knows more than the characters.In Romeo and Juliet the servants have to run round setting up the stage to start the next scene. We cannot be here and there too. This is because Romeo Juliet was written in 1595 and was set in the Globe theatre. There was not many props and equipment in Shakespeares day li ke there are today due to the fact that they never had much money. At the start of the scene the mood is happy, cheerful and tense. Lord Capulet is in a happy mood at the start of the scene because he is trying to marry his only daughter Juliet and his party is going well. He is happy and polite to his guests at the party talking in a poetic verse to show his importance. Welcome, gentlemen!In lines 53-91 there is a dramatic change in mood compared to the peaceful mood the audience previously see. The audiences tension and excitement is increased when Tybalt over hears Romeo talking. This by his voice, should be a Montague. He fills up with rage and shouts Fetch me my rapier. With this the audience are sitting on the edge of their seats. Romeos arrival changes the atmosphere and the audience are aware that it becomes tenser this is because the audience knows that Montagues arent welcome at a Capulets party.The scene is also romantic because this is the first time Romeo sets his eyes on Juliet and falls in love with her. For I neer saw true beauty till this night. Tybalt hears his voice and becomes angry Now by the stock and honour of my kin to strike him dead I do not sin. He goes and tells Lord Capulet that Romeo is here at his party. Tis he, that that villain Romeo. Lord Capulet is annoyed at him because he doesnt want anything to happen to his guests. Lord Capulet tries to calm Tybalt down, he tells Tybalt To say truth, Verona brags of him to be a virtuous and well governd youth. Tybalt becomes even more enraged; Ill not endure him, with this Lord Capulets attitude and manner suddenly changes towards Tybalt. He shall be endurd. With this Tybalt storms out of the room leaving the audience in suspense.When Romeo ; Juliet first meet this is vitally important in the rest of the play because if they didnt meet then the rest of the play wouldnt have happened. Romeo is very dazzled by Juliet and knows it is love at first sight. He expresses his inner feelings in a soliloquy which is controlled by his devotion and admiration for Juliet. O she doth teach the torches to burn bright? Shakespeare then uses a sonnet form with rhyming couplets to emphasise the love and romantic nature of the scene. So shows a snowy dove trooping with crows as yonder lady oer her fellow shows When Romeo and Juliet speak to each other the speech forms a sonnet; this is the 14 line poem which has alternate rhyme and ends in a rhyming couplet and this shows us the way they feel about each other. Did my heart love till now?. They start to talk about kissing. My lips two blushing pilgrims, ready stand to smooth that rough touch with a tender kiss. The audience knows that this is going to lead to something and know that they are about to share a kiss, which they do. Good pilgrim, you do wrong your hand to much. This shows religious images and also shows how deep and pure their love is. Romeo is in a world of his own, enraptured by the stunning beauty of Juliet. This creat es a romantic but also dangerous mood in the scene as the audience watch with anticipation and ask will they be caught?At the end of the scene, Shakespeare uses hints to make the audience think about future events in the play. Here Romeo and Juliet find out who the other actually is. After they exchange their love Juliet asks the nurse whom Romeo is, the nurse replies, His name is Romeo, and a Montague the only son of your great enemy. Juliet is so upset and shocked by this and says in confidence to the nurse, My only love sprung from my only hate! Too early seen unknown, and known too late! She now knows that he is a Montague and knows her family wont accept their love for one another.At the end of the scene I think that the audience become more anxious as they want to know what else happens in the scenes to come. If they did a play for a modern audience I think that it will work as it did in Shakespeare time because everyone enjoys love stories and they will have more equipment and props so the play will become more realistic.This scene is very important to the play because without it lots of other events wouldnt take place. This scene also has dramatic irony because the audience knows more than the characters as the audience are fully involved in the play. I like this scene in the play because I like the way Romeo and Juliet first meet and fall in love, I also like the way Shakespeares language is used to create characters, to add emotion and express meanings in the scene.

Comparison between 17th & 18th centuries in France,England and America - Term Paper Example The first concept that can be seen with the different pieces of architecture is the characteristics that are from each region. The first comparison which can be made is within England, specifically between the 17th and 18th centuries. In the 17th century, the style was based on the Baroque influences, which consisted of finding balance within each type of work. This was combined with the styles which were influenced by the political and cultural associations, inclusive of the Tudor monarch changing to a different political structure. The main concepts which were associated with the Baroque style began to change by the 18th century, specifically because of social and cultural associations. The political realms moved through revolutionary changes, specifically with an altered regime and order. This was combined with the Enlightenment and the Industrial Revolution, both which altered the materials used for architecture as well as the expectations of what should be included in the archit ectural styles. Many of the pieces of architecture began to change into experimentation with new materials, influences of structures that were romantic in style and which rebelled against the balance and form of the 17th century (Glennie, p 17). The examples of these changes show in the variety of architecture which was produced in England during the 17th and 18th century. The earlier work of the Hatfield House (1607) shows the Elizabethan style as well as how this related to the metaphor of the political and cultural structure, as seen in Figure 1. Figure 1: Hatfield House (1607) The architecture is one which is representative of the Elizabethan architecture first because of the distinct form of east and west wings as well as the central arches which remain even on both sides. The towers and the association with simplistic and ornate figures are also seen. The main concept was to keep an open region while offering a spacious area that represents the power of the political structure of the time. This was representative of the English time frame and the components which were associated with the 17th century balance and structure (Archer, 29). The changes in England can be seen with the progression of architecture, such as through the Chiswick House, (1725) seen in figure 2. Figure 2: Chiswick House (1725) The same references of balance are used with this, specifically with the association of columns and structure that was used. This is not only representative of the ideologies of the new political system and the thriving of the current status. Instead, there is a movement back to the ancient Greek structures for balance, seen through the columns and the dome that is used. The concept of architecture is one which combines several influences and uses experimentation of combining structures while keeping the balance as seen in the 17th century. The representation then shows the change in style with integration of different methods, specifically which is representa tive of the characteristics of the Industrial Revolution while keeping the same concept of balance and form as the central component of the time, which is specific to Neo – Classicism (Archer, 37). The movement from balance and into a sense of Romanticism was also seen in other regions, including France and Colonial America. Walden and Transcendentalism Essay Henry Thoreau’s masterpiece, Walden or a Life in the Woods, shows the impact transcendentalism had on Thoreau’s worldview. Transcendentalism is a philosophy that asserts the primacy of the spiritual over the material. Transcendentalism puts the emphasis on spiritual growth and understanding as opposed to worldly pleasures. Thoreau’s idea of transcendentalism stressed the importance of nature and being close to nature. He believed that nature was a metaphor for spiritual enlightenment. A walk in the woods therefore was a search for spiritual enlightenment. One should look ‘through’ nature, not merely ‘at’ her. In Walden, Thoreau’s idea of transcendentalism is broken into three areas. The first is the importance of the spiritual world as opposed to material wants. He accentuates this idea by explaining how the physical world only exists so that souls can experience life to the fullest. Thoreau speaks a great deal about physical property in the first chapter, entitled â€Å"Economy†. He keeps a detailed record of the economic cost of his venture into the woods and explains to his readers his pity for the people who have numerous material possessions. Thoreau states, â€Å"When I have met an immigrant tottering under a bundle which contained his all†¦I have pitied him, not because that was his all, but because he had all that to carry. †[1] The second transcendentalism theme is the idea of individuality- the idea that an individual is unique and should act according to his personality and ideals. Individuality is a basic idea of the transcendentalists and they firmly believed that one should search for ‘self-discovery’. Thoreau observed, â€Å"Every man is the builder of a temple, called his body, to the god he worships, after a style purely his own, nor can he get off by hammering marble instead. †[2] He emphasized the â€Å"style† as an individual style, one that was distinctive to each person. He even told his readers in Walden that he went into the woods to search for knowledge and truth. Self-discovery and individuality were also attributed to any other characters in Walden that were mentioned. Thoreau takes great pains to describe each character, even down to the farmer’s â€Å"wrinkled, sibyl-like, cone-headed†[3] infant in chapter 10, â€Å"Baker Farm†. He makes sure his readers understand the unique attributes of each individual in his experiences. As Thoreau once said, â€Å"It is what a man thinks of himself that really determines his fate. † The final prominent transcendentalism theme expressed in Walden is the importance of experiences. Transcendentalists believed personal experience is how one learned. Literally, people learned everything the hard way. Thoreau demonstrated this clearly in the experiment of living in the woods for two years. He explains in Walden that he wanted to experience living simply for an extended period of time. Notice that Thoreau did not speculate, draw conclusions, or even ask someone who had tried it. The only way, in his mind, that he was going to learn about living simply was to undergo it personally. In speaking about life in the chapter, â€Å"Where I lived and What I Lived For†, Thoreau said,â€Å"†¦if [life] proved to be mean, why then to get the whole and genuine meanness of it, and publish its meanness to the world; or if it were sublime, to know it by experience and be able to give a true account of it in my next excursion. †[4] He emphasized living and feeling everything to be able to understand life and its meaning. Walden is often viewed as simply a proponent of environmental care and nature. However, it persuades the reader to do much more than take care of nature. In fact, nature is not even the most prominent ideal in Thoreau’s writings. The thesis of Walden is clearly stated in the first few pages of the book. Thoreau writes, â€Å"With respect to luxuries and comforts, the wisest have ever lived a more simple and meager life than the poor†¦None can be an impartial or wise observer of human life but from the vantage ground of what we should call voluntary poverty. †[5] The entirety of the â€Å"Economy† chapter is devoted to material possessions and Thoreau’s idea that the physical pleasures exist only to help the soul endure. Very little time is spent on the goodness of nature. When it is mentioned, it is shown, as was stated above, that nature serves as a sort of looking glass to spiritual ideals. Because this book was quoted often by radical groups in 1960-70, Walden became a sign of disobedience and rebellion to the conservative community. However, there are a few ideas of which Christians can approve. The first is non-materialism. Thoreau quotes Matthew 6:19 saying, â€Å"By a seeming fate, commonly called necessity, they are employed, as it says in an old book, laying up treasures which moth and rust will corrupt and thieves break through and steal. It is a fool’s life, as they will find when they get to the end of it, if not before. †[6] He takes great pains throughout the entire book to make sure his readers understand that material possessions should not be the only thing in which people place all of their happiness. The second idea Christians can applaud is the idea of individuality. As was mentioned above, Thoreau kept the Transcendentalist idea of a person’s individual worth in his writings. Because of this belief, he was one of the most outspoken human rights activists in his lifetime. He wrote A Plea for Captain John Brown supporting John Brown’s efforts to end slavery. Thoreau said, â€Å"I do not believe in erecting statues to those who still live in our hearts, whose bones have not yet crumbled in the earth around us, but I would rather see the statue of Captain Brown in the Massachusetts State-House yard, than that of any other man whom I know. I rejoice that I live in this age, that I am his contemporary. †[7] His belief in the basic human rights of every man stemmed from his support of individuality and the unique worth of every person on this planet. Even though there are a few ideas that Christians can applaud in Walden, there is one of which they should be wary. This idea is the idea that rebellion and disobedience towards government is acceptable if one believes the government is wrong. Ralph Waldo Emerson, in his eulogy for Thoreau, articulated this idea, saying that Thoreau opposed the government. Thoreau disrespected government officials by refusing to obey tax laws and paid for it by spending a brief time in jail. However, many people, instead of realizing the negative influence Thoreau was creating, idealized him for his ‘patriotic’ stance. Thoreau says in Walden, â€Å"One afternoon, near the end of the first summer†¦I was seized and put into jail, because, as I have elsewhere related I did not pay a tax to, or recognize the authority of, the state which buys and sells men, women, and children, like cattle at the door of its senate-house. †[8] He did not recognize the authority of the government because of the slavery in the country. Even though slavery is immoral, Romans 13:1 clearly states, â€Å"Let every person be subject to the governing authorities. For there is no authority except from God, and those that exist have been instituted by God. †[9] The Bible articulates that Christians must be subject to those whom God has placed into authority. Thoreau discourages this idea and instead institutes an attitude of rebellion. Christians should be cautious of this attitude and worldview. Very few instances occur in which Christians should rebel against authority. The only instance where they should rebel is under a government which demands that its citizens disobey God’s orders. Walden was written many years ago and yet, society can still learn from the author’s words. Whether the ideas are detrimental or helpful, everyone can agree that Thoreau was a strong Transcendentalist with a distinct mindset. This mindset affects everyone who reads his works. Emerson once articulated that Thoreau inspired many around him through his idealistic thinking. Thoreau has inspired, and will continue to inspire, numerous people through his book, Walden or a Life in the Woods.
